Because the condition she suffered from wasn't anything the hospital could help with. It was some
sort of mystery illness that stumped doctors to the point there was just nothing that could be
done.
All things considered, taking her out into the snow did seem retarded, but the stipulation was
that she was gonnersville anyway and she wanted to see something one last time before she died.
Tomoya didn't want to take her but once he realized the situation he went along with it anyway.

The problem with the Anime is it doesn't convey how it worked as well as the Visual Novel did.
Throughout the game, though each arc, Tomoya finds a glowing sphere. If enough of these spheres
are collected, they have the power to change what happened. By the very end of the game/show, he
collected enough of the spheres to change Nagisa's death.
