It's clearly self harm, people with those types of disorders commonly isolate and personify the

And also the game implies he has no friends, where exactly do you think he'd be getting heroin
from? Last I checked, it doesn't grow on trees, and it certainly ain't easy to get.
Given that the game is a piece of abstract bullshit altogether, based soley off the line you
posted, self mutilation makes a lot more sense.
Many cutters start off cutting around their forearms/biceps where it can't be seen with a
t-shirt. If she "stuck there ever since" then it likely left a scar which isn't going to vanish
anytime soon.

From my ending it was made pretty clear, that it can't be heroin. After waking up in hospital the
nurse remarked "Thank god you didn't hit any nerves or arteries. - Why did you do it?". That
would make no fucking sense with a drug injection.
