Yes, you have to ask for permission to leave the classroom.
Hall passes were there so that hall monitors (lol) knew you weren't cutting class.
Thinking about it now, there's no way Hall Passes were sanitary if little kids were taking them
into the bathroom with them.

Segregated PE depends on what school you go to. Prior to high school and in small schools they
generally have it together, elsewhere they divide the gym by gender. There's also all-male and
all-female schools. There are no hard rules.

In my country, there are single sex PE classes since forth grade of primary school (10-11 year
olds). It's really hard for me to imagine having PE along with the opposite sex. Playing sports
would be okay, but I don't know about the stretching exercises, fitness and yoga stuff... It must
be really embarassing.

All of the schools in my home town were also segregated PE class - though apparently after
someone picked the lock between the change rooms, the Catholic school had co-ed showers after gym
for a while. Then again, that was also the school that had to fire two teachers for getting
caught having sex in a janitor's closet by the students during lunch. (They were getting taught
all of that "morality" stuff that us heathen public school kids didn't learn.)

Depends on what school you go to.
It's required that you get a pass in all public schools,
but in high school it depends on what teacher you get.
Some don't really care and you just pick up a pass and go
without saying anything.

The gender segregation happens on its own, they don't enforce it.
To us it's not that big of a deal to exercise with the other gender cough.

I went to a poor rural high school...
No co-ed gym classes after grade 8.
No lockers in the change rooms (lots of theft).
No cafeteria in jr.high (we had to eat while sitting on the gym floor).
No swimming in any gym classes.
No football team (we had a field but the school could not afford football equipment or insurance).
They tried having a cheerleader squad one year, but nothing to cheer at.
But we had no hall passes or hall monitors.
