The third secret is on level 11. Once you make it to the end, don't go through the door but
instead jump down. You'll fall to the previous screen and land at another door. Go into that door
and collect your prize.
Bonus ending is pretty shit though.

Newfag guide-
Secret 1 - (L6)
Continue jumping left past the door, falling down the pit beyond the block.
Secret 2 - (L7)
Near the end, after jumping back on the moving platform, jump up to the door to avoid hitting the
side of the block, and jump back on the platform once more, riding it off the screen.
Secret 3 - (L11)
After completing the main challenge, ride the floating platform to the exit door, however, fall
off the left side to a door below, and collect the final treasure
