You decide not to hurt your brother and die.

There's a disease actually where a child is born with two sets of cells, one of which belongs to
his nearly identical twin. The body not knowing what to do starts killing itself trying to get
rid of the intruder without knowing what it truly is.
I forgot its name though, and I'm not too big into medicine. Looking it up, brb.

Can't find the thing I was talking about, the closest I could find is a teratoma, basically it's
a parasitic twin without a spine and still in the middle of embryogenesis. Here's the weird part
about it though, these "tumors" can grow anywhere in the body, and the tissue and organs it
produces varies greatly.

Chimerism. A chimera is born with two sets of cells, from where one cell absorbed its twin.
Sometimes the resulting person can live normally, but it sometimes causes problems. Examples:
People with multiple body parts (two arms, two spines, etc.)
Hermaphroditism
Random allergic reactions only in certain parts of body
If legitimacy of children comes into play, and a DNA sample is taken from one part of the body,
the test will turn up negative when the child was legitimate.

There's a condition knows as Fetus in Fetu where there's two cells. basically twins, but as they
develop somehow one envelops the other. The one that envelops becomes a basically normal baby,
and the twin remains inside their sibling. as the big one gets older, sometimes the enveloped one
will grow too, feeding off the host like a tumor and will almost develop into a fully formed
fetus, or just really disformed. they usually have no brain tissue or other organs
