While I understand the tie-in, the nurse says "So why did you do it?" which makes it seem as if

That, and you don't delve deeper into heroin (or similar addictive drugs). You can get addicted
to the drug and frequent it, you can dose higher and get more dramatic effects, but you're never
any closer to succumbing per-se than you were the last time.
