A lot of people thought this was about heroin the last time this was posted, but I'm almost
certain this is a metaphor for cutting. When you do the "I really like you" ending you end up in
the hospital. The nurse asks you "so why did you do it". And the boy narrates how it was easy to
fool the doctors into having them think he was ok.
