it's a subcutaneous tissue abcess, caused by use of something called synthol, a 'cosmetic muscle
enhancer' that attaches itself to muscle and some fatty tissue and causes it to puff up, giving
the illusion of large muscles. The infection spreads rapidly through the weakly protected layer
of tissue just below the skin, and just above the muscle, slowly eating away at the muscle tissue
below. Eventually it ends up looking like this. He's fortunate it didn't do more damage.
